Home Health Aide (HHA) – Resident TransportJoin Our Team as a Home Health Aide at The Phoenix Rehabilitation and Nursing Center. The Phoenix Rehabilitation and Nursing Center is seeking a reliable and compassionate Home Health Aide (HHA) to assist with the safe transport of residents within and outside the facility. This is a vital role that ensures residents arrive safely to appointments, activities, and therapy sessions while providing friendly and supportive care.This position is primarily focused on resident transportation, making it ideal for candidates who enjoy working directly with seniors and supporting their mobility and well-being.Home Health Aide (HHA) ResponsibilitiesSafely transport residents to and from medical appointments, therapy sessions, and facility activitiesProvide assistance and support to residents during transfers and mobility challengesMonitor and ensure resident safety and comfort during transportCommunicate effectively with the care team regarding resident needs and observationsAssist with light resident care and companionship as neededFollow all facility policies, procedures, and safety guidelinesHome Health Aide (HHA) QualificationsHHA certification in good standingValid driver’s license and clean driving record (if transporting offsite)Ability to lift and assist residents safelyStrong communication and interpersonal skillsCompassionate, patient, and resident-focusedPrior experience in a skilled nursing or rehabilitation setting preferred but not requiredPay Rate: $18 per hourWhat We Offer$19 per hourFlexible schedulingOpportunities for growth and development within the facilitySupportive and team-oriented work environmentIf you are a dedicated Home Health Aide who enjoys helping residents and ensuring their safety and comfort, we encourage you to apply for this position at The Phoenix Rehabilitation and Nursing Center.An Equal Opportunity Employer
