Home Health Aide (HHA)As a Home Health Aide (HHA) with SYNERGY HomeCare of Woodbridge, you will play a vital role in delivering high-quality care to patients in their own homes. Our team is dedicated to providing compassionate and personalized support to individuals in need, and we're looking for a compassionate and skilled HHA to join our family.Responsibilities:Provide personal care services such as bathing, dressing, and grooming to patients in their homesAssist patients with daily living activities, including meal preparation, light housekeeping, and medication remindersMonitor and report changes in patient condition to healthcare professionalsMaintain accurate and detailed records of patient care and services providedCollaborate with healthcare teams to develop and implement patient care plansDemonstrate empathy and understanding towards patients and their families, promoting a positive and supportive environmentRequirements:High school diploma or equivalent requiredCurrent certification as a Home Health Aide (HHA) in the state of New JerseyAbility to work independently and as part of a teamExcellent communication and interpersonal skillsAbility to lift, transfer, and move patients safely and comfortablyValid driver's license (if applicable)About SYNERGY HomeCare:At SYNERGY HomeCare, we build connections and forward momentum in people's lives.
