Home Health Aide (HHA)
As a Home Health Aide (HHA) with SYNERGY HomeCare of Woodbridge, you will play a vital role in delivering high-quality care to patients in their own homes. Our team is dedicated to providing compassionate and personalized support to individuals in need, and we're looking for a compassionate and skilled HHA to join our family.
Responsibilities:
Provide personal care services such as bathing, dressing, and grooming to patients in their homes
Assist patients with daily living activities, including meal preparation, light housekeeping, and medication reminders
Monitor and report changes in patient condition to healthcare professionals
Maintain accurate and detailed records of patient care and services provided
Collaborate with healthcare teams to develop and implement patient care plans
Demonstrate empathy and understanding towards patients and their families, promoting a positive and supportive environment
Requirements:
High school diploma or equivalent required
Current certification as a Home Health Aide (HHA) in the state of New Jersey
Ability to work independently and as part of a team
Excellent communication and interpersonal skills
Ability to lift, transfer, and move patients safely and comfortably
Valid driver's license (if applicable)
