Home Health Aide (HHA/PCA) Rensselaer County, NY
At Home Care Partners is a Licensed Home Care Agency committed to helping individuals remain safe, comfortable, and independent in their own homes. We provide compassionate, high-quality care while supporting both our clients and their families.

Your Role
As an HHA/PCA, you will provide valuable one-on-one assistance to clients in their homes. Your care and support will help clients maintain their daily routines and continue living as independently as possible.
Responsibilities include:
Helping clients with bathing, grooming, and dressing
Assisting with meals and light housekeeping
Providing companionship and daily support
Giving medication reminders
Reporting changes to the care team
Helping maintain a safe home environment
