Home Health AideLakeview is looking to hire a Home Health Aide to join our Homecare/Hospice team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.Position Summary:Assists professional personnel to provide patient care according to department and hospital standards. Completes other supportive tasks necessary to the functioning of the department under the supervision of a Case Manager.Essential Duties:Demonstrates competence in performance of tasks.Vital signsWeigh patientsTransfer patient safelySpecimen collectionAseptic TechniqueClean dept specific equipment and suppliesDocuments accurate vital signs and I & OAssists patients with ADLs and performs other tasks as assignedFoley catheter careSimple dressingsPrepare meals, feedObserve safety issues in the homeApply teds, ace wraps, slings etc.Bathing with or without assistanceFoot soaksShampoo hairFile fingernails and toenailsCombing/brushing hair, makeup and shavingPerform personal hygiene tasks clients are unable to perform without assistancePerform total bath, requiring turning and repositioningPerform perineal carePerform skin care measures, e.g. application of barrier cream or lotionProvide for oral /teeth care.Perform or assist the client in performing activities related to mobility-passive range of motion, turning and repositioningDo routine ostomy cares e.g. change or empty the bag only. No wafer caresPerform or assist the patient in performing the normal activities of daily living such as eating and dressingAssist the patient in taking self-administered oral medicationsPerform light housekeeping duties to maintain the patient's level of function: prepare light meals, change bed linens, clean and straighten the patient's immediate personal environment.Report significant observations and changes in the patient's condition to the CM within a timely manner.Oxygen per patient request.2. Communicate pertinent data clearly, concisely and in a timely manner to the Case Manager.3. Maintain department order and equipment management as needed.4.Performs other tasks as required or assigned.5. Point of care documentation into EHR.6.Driving to client's residence.Work Schedule:0.6 FTE (scheduled 3 days per week). Working hours are 8-4:30 pm. No weekends or holidays.Required Qualifications:Education High school diploma or equivalentLicense and Certification: Must have a valid driver's license and meet all criteria required under Lakeview's Automobile Liability GuidelinesMust have MN or WI CNA certificate upon start dateMust provide proof of MN and WI CNA certificates within 4 months of start, if both MN and WI CNA certificates are not present on start dateCurrent BLS – American Heart Association or BLS – American Red Cross (CPR/AED for Professional Rescuer & Health Care Provider)Experience / Skills Must be 18 or older and have at least 1 year of driving experiencePreferred Qualifications:Experience / Skills Prior nursing assistance experienceBenefits:Health Partners offers a competitive benefits package (for eligible positions) that includes medical insurance, dental insurance, a retirement program, time away from work, insurance options, tuition reimbursement, an employee assistance program, onsite clinic and much more!
