Job Title: Home Health Aide
Service Location: Pittston, PA
Rate: $14-$16/hr
Immediate Need:
Pittston, PA - Monday - Thursday 8am-3pm/Friday 3pm-11pm/Weekends 8am-3pm & 3pm-11pm

Responsibilities: Performs personal care/ household duties (e.g. dressing, toileting, feeding, light housekeeping) in home/school setting
Maintain a clean and safe environment
Provide emotional/behavioral support
Follows plan of care
Ability to interact as a member of a multidisciplinary team
Reports any changes in the patient condition to agency/nurse case manager
