Job description
Home Health AideWe are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.ResponsibilitiesHelp the client to maintain good personal hygiene and assists in maintaining a healthful, safe environment.Encourage the client to become as independent as possible according to the care plan.Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisorBuild relationships with patients by conversing or reading aloud to help keep them mentally healthy and alertPromptly report any change in client condition to the Supervisor.QualificationsGraduated from an accredited Home Health Aide programHigh School Diploma or GEDOne year prior professional experienceDriver's license requiredCPR certification requiredWe are an equal opportunity employer and all qualified applicants will receive consideration for employment without regard to race, color, religion, sex, national origin, disability status, protected veteran status, or any other characteristic protected by law.
