Job description
We are seeking compassionate and dependable Home Health Aides (HHA) and Personal Care Aides (PCA) to provide supportive care for patients in their homes. This role is ideal for caregivers who are passionate about making a meaningful difference during a patient’s end-of-life journey.
nResponsibilities:
nnProvide personal care assistance including bathing, dressing, grooming, and mobility support
nOffer companionship and emotional support to patients and families
nAssist with light housekeeping and meal preparation
nObserve and report changes in patient condition to the care team
nMaintain patient comfort, dignity, and safety at all times
nFollow hospice care plans and agency guidelines
n nQualifications:
nnActive New York State Home Health Aide (HHA) certification required
nHospice experience preferred but not required
nCompassionate, patient, and professional attitude
nStrong communication and interpersonal skills
nValid driver’s license and reliable transportation preferred
nAbility to travel between patient homes
