Home Health Care Aide

nResponsibilities - Include but not limited to
n** Home Health Aide (HHA) – Personal Care Assistant (PCA)**
n· Ensure quality and safe delivery of home health care services.
n· Implement client’s plan of care in accordance with the Agency’s process and procedures policy.
n· Participate in Client’s case conferences according to the Agency’s policies and regulations.
n· Submit updates and relevant information regarding the Client’s plan of care to the Registered Nurse in a timely manner.
n· HHA/PCA will work with patients of all ages that require care in their homes.
n· HHA/PCA must be able to carry out patient care plan activities.
n· HHA/PCA must be caring, patient and compassionate caregiver.
n· HHA/PCA must be able to accompany patients to doctor’s offices or to hospital visits.
nQualifications
n· High School Diploma or Equivalent – Preferred.
n· Home Health Aide certification required.
n· 1 year Home Health Care experience. Experienced Preferred.
n· Able to read, understand, and execute instructions.
n· Ability to communicate and write effectively.
n· Ability to work and deal within a stressful environment.
n· Bilingual – Spanish/Creole/etc. (Preferred - Not Required) - Must also be able to read/write and speak English.
nCompensation and Schedules
nnnSchedules – Varied (AM/PM/Overnight shifts/Live-in available).

nnMultiple locations available we cover Brentwood, Bayshore, Islip and more!

nnCompensation - $19.75 plus benefits.
