Job description
Gentiva is seeking an RN On Call to provide after-hours home-based clinical support on a 7-on/7-off schedule. In this role, you’ll triage calls, conduct urgent visits, manage symptoms, and support hospice and home health patients in their homes. You’ll assess changing conditions, collaborate with on-call providers, adjust care plans, and ensure timely, accurate documentation. Working independently in the field, you’ll provide education, emotional support, and compassionate end-of-life care that honors patient dignity while partnering closely with an interdisciplinary care team.
