Job description
VitalCaring seeks a Clinical Care Manager RN to lead home health and hospice care for patients across the Dallas area. In this role, you will manage a caseload of medically complex patients, complete comprehensive assessments, and develop individualized care plans focused on safety, recovery, symptom control, and quality of life. You will coordinate interdisciplinary services, support safe transitions of care, and collaborate closely with physicians and families.
