Job description
Innovive Health in Gloucester, MA seeks an experienced Registered Nurse Case Manager to deliver and coordinate skilled nursing care at patients' homes. You will work with the Clinical Director and care team to implement physician orders under policy and regulatory standards.
Responsibilities include initial assessments, care planning, medication management, and teaching patients and families. A valid RN license, NCLEX, CPR, and 1–2 years in home health preferred.
