Description
Up to a $10,000 sign on bonus!
40 hrs. per week. Monday - Friday 8:00am - 4:30pm. No weekend requirement but holiday requirement per agency guidelines.

About the Job:
Admission to HHHC's homecare division.
Completes an initial home health assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
Utilizes the nursing process in planning, providing, and evaluating skilled needs.
Completes a comprehensive patient assessment that is reflected in accurate OASIS scoring.
Develops a care plan that establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and family on the planning and revising process.
Who You Are:
Graduate of an accredited school of nursing.
Associates degree in nursing.
Registered as RN with New Hampshire’s Board of Nursing.
A minimum of one (1) year recent acute care experience.
Ability to exercise initiative and independent judgment.
Excellent verbal and written communication skills and competent in problem solving abilities.

Education
Required
Associates or better in Nursing.
Preferred
Bachelors or better in Nursing.
Experience
Required
1 year:
1 yr. acute care experience in med/surg or acute rehab preferred
Licenses & Certifications
Required
Drivers License
CPR Certification
Registered Nurse
