Job description
MJHS Home Care in the Greater New York area seeks a skilled Registered Nurse to coordinate and deliver advanced home health services, including patient assessment, medication management and patient education. You will develop and update the interdisciplinary plan of care with the physician, case manage the care of assigned patients to optimize outcomes, and ensure thorough documentation.
Requires NYS RN license, at least 1 year acute medical‑surgical experience; CHHA experience preferred; valid
