Job description
The Home Health Certified Nursing Assistant provides in-home support to clients, following an established plan of care. Responsibilities include personal hygiene, assistance with transfers and mobility, basic nursing care under RN or therapist supervision, meal prep and light housekeeping, companionship, accurate documentation in EVV or timesheets, and reporting changes in condition or safety concerns. The role requires reliable transportation, frequent travel between client homes, and adherence to patient privacy and safety standards.
