nWe are looking for a per diem Certified Occupational Therapist Assistant (COTA) with recent Home Health experience to visit patients in their homes in Moreno Valley and Riverside area.

nResponsibilities and Duties:
nAssess patient's condition under a PT during every visit and chart observations
nPerform evaluation tasks, including vital signs
nFollow prescribed treatments, following plan of care and assess progress of healing
nProvide appropriate timely feedback to Physical Therapist and other professionals
nCharting and documenting
