About the Role
Sierra Health Care is looking for a Clinical Manager to lead home health clinical operations in Las Cruces. In this role, you'll oversee a team of nurses and therapists, guide individualized patient care planning, and make sure every patient receives safe, coordinated, and compassionate care. Key Responsibilities
Supervise and support nursing and clinical staff to ensure safe, patient-centered care delivery
Recruit, orient, and evaluate clinical staff, including ongoing competency validation
Oversee individualized patient care plans and lead care conferences
Coordinate closely with physicians, community partners, and the interdisciplinary team
Lead quality and compliance efforts through QAPI, infection prevention, and emergency preparedness
Ensure timely, accurate clinical documentation in the EMR
Stay available for consultation and on-call coverage to support continuity of care
Manage clinical staffing, scheduling, and resources to keep operations running smoothly
Requirements
What We're Looking For Graduate of an accredited nursing or therapy program; current, unrestricted New Mexico license required
Minimum 3-5 years of clinical experience, including at least 1 year in home health or a related setting
Demonstrated ability to supervise clinical staff, coordinate care, and maintain regulatory compliance
Strong knowledge of Medicare and Medicaid Conditions of Participation and state licensure standards
Proven leadership, communication, and organizational skills
EMR proficiency and comfort managing multiple clinical priorities
Valid driver's license and reliable transportation
