nAs a Clinical Team Specialist, you’ll contribute to our success in the following ways:
nnnProvides oversight of all patient care services including coordinating patient care, coordinating referrals, assuring patient needs are continually assessed, and assuring the development, implementation, and updates to the individualized patient plan of care.
n
nnReviews and approves plan of care and evaluates proposed changes to the plan of care for clinical appropriateness.
n
nnConducts regular OASIS reviews for all patients.
n
nnEnsures required visits are completed timely.
n
nnReviews plans of care and other paperwork for accuracy and compliance with State and Federal requirements
n
nnMonitors timeliness of required documentation to be uploaded to HCHB by direct care team members and LPN/LVN Clinical Coordinators, and maintains records and required documentation, assuring accuracy, completeness and compliance with licensing regulations, certifications standards and legal and ethical imperatives.
n
nnParticipates in the organization’s strategic planning and QAPI activities.
n
nnPerforms other duties/projects as assigned.

n nWhat is Required?
nnnAssociate degree in a Nursing related field required
n
nn2 years home care experience as a registered nurse in a Home Health
n
nnCurrent, unrestricted RN license for Massachusetts
n
nnAbility to quickly become proficient in enterprise applications such as Homecare Homebase, Workday, and have knowledge in Microsoft Office
n
nnMay be asked to travel on occasion to support a field office in time of audit or necessitating event.
n
nnMust have a dependable vehicle, valid driver’s license, and current auto insurance in accordance with state laws.
n
nnAbility to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
n
nnWill be required to attend branch staff meetings quarterly
n
