nnShift Hours: 8:00 AM - 5:00 PM / Monday - Friday / 1 Weekend Rotation a month.
n
nnWeekend Work: Occasional

nnOn Call: Yes
n
nnHome visits in assigned service area: Hillsborough County, FL
n
n nThe Physical Therapist provides hands-on care in the home for a group of patients consistently in accordance with the principles and practices of physical therapy. Responsibilities include :
nnnEvaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of Physical Therapy.
n
nnResponsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
n
nnPlans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patient's needs are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
n
nnManages the activities of the multi-disciplinary team providing care to the patient.
n
nnMonitors the provision of therapeutic services through written and verbal communication with all disciplines.
n
nnDevelops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
n
nnInterprets agency policy to patients/families.
n
nnReviews patients visits weekly and provides plan to PCS and team assistant, including anticipated weekend visits.
n
nnInforms team assistant of any schedule changes and communicates caseload and patient care issues to Patient Care Supervisor
n
n nRequired Certifications and Licensures:
nnnBLS (Basic Life Support)
n
nnPT (Physical Therapist) License
n
nnDriver's License State of Florida (Florida driver's license is required within 30 days of hire)
n
n nEducation:
nnnRequired Bachelors (Physical Therapy)
n
nnPreferred Masters (Physical Therapy)
n
n nExperience:
nnnRequired 1 year Physical Therapy
n
nnPreferred 1 year Home Care
n
n nThis position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at .
