Job description
Choice Health at Home is seeking a Home Health Field RN (Northern Phoenix) for a full-time position at the Scottsdale Home Health Branch in Scottsdale, AZ. The RN conducts comprehensive home health assessments and coordinates patient-centered care in the home setting.
The role requires an active RN license, CPR, and a valid driver’s license, with 2+ years Home Health RN experience preferred. HomeCare HomeBase experience preferred. Benefits include a 401k plan after 3 months and immediate vesting.
