Job description
As an Intake Coordinator at Addus Family Company, you will be the first point of contact for individuals seeking home and community-based care. In this client-facing role, you collect and verify referral, clinical, and insurance information, complete eligibility checks, and accurately enter data into intake and EMR systems. You will coordinate start-of-care activities with clinical and scheduling teams, communicate clearly with patients, families, and referral sources, and ensure timely, compassionate service that supports clients living safely and independently at home.
