Job description
VitalCaring seeks a Physical Therapist to deliver high-quality home health and hospice rehabilitation services. You will assess functional status, develop individualized care plans, provide therapeutic interventions, and educate patients and caregivers to improve mobility, safety, and quality of life. Collaborate closely with nurses, aides, and physicians, using evidence-based practice and strong documentation skills.
