As a Home Health Intake Coordinator, you'll serve as a vital connection between patients, families, referral sources, and clinical teams, ensuring referrals are processed accurately, admissions are coordinated efficiently, and patients receive the care they need without unnecessary delays. Your attention to detail and commitment to exceptional service will help create a positive first impression while supporting continuity of care.This role is ideal for someone who is highly organized, customer-focused, and enjoys coordinating multiple priorities in a fast-paced healthcare environment. You'll manage patient intake and data entry, verify referral information, audit admission documentation, coordinate scheduling, communicate with patients and community partners, and monitor documentation to ensure timely admissions and regulatory compliance.
