Job description
CenterWell Home Health is seeking a compassionate Licensed Practical Nurse to deliver skilled care to patients in their homes. In this role, you’ll provide direct nursing services, administer medications, perform wound care, monitor vital signs, and support care plan implementation under RN supervision. You’ll educate patients and families, promote safety and independence, and coordinate with an interdisciplinary team.
