Job description
Well Care Home Health of the Triad, Inc. seeks a Licensed Practical Nurse to provide skilled home health care to patients across the Greensboro area. The LPN delivers compassionate, evidence-based nursing, including assessments, wound care, medication administration, disease management education, and coordination with RNs and therapists. This role offers autonomy in the field, supportive clinical leadership, and flexible scheduling. You will document visits accurately, communicate changes in condition, and collaborate with the care team and families to promote safety, independence, and optimal quality of life in the home setting.
