Job description
Professional Case Management is seeking a compassionate Licensed Practical Nurse to provide in‑home care for clients with complex medical needs, including former energy workers and their families. In this role, you will deliver skilled nursing services such as assessments, wound care, medication administration, and monitoring of chronic conditions. You will collaborate closely with RNs, physicians, and caregivers to execute individualized care plans, educate clients and families, and document all care accurately.
