Job description
CenterWell Home Health, part of Humana Inc., is seeking a Home Health Licensed Practical Nurse to deliver compassionate in-home care to seniors and adults with chronic conditions. In this field-based role, you’ll provide skilled nursing services, including medication administration, wound care, and patient education, while closely monitoring patient status and documenting in an EMR. You’ll collaborate with an interdisciplinary team to support safe recovery, manage illness, and promote independence at home in a culture built on respect, teamwork, and professional growth.
