Job DescriptionJob DescriptionHome Health LPN – 13-Week Travel Contract
KPG Healthcare is seeking an experienced Home Health LPN for a full-time 13-week travel assignment. The LPN will provide skilled nursing care to patients in their homes, completing scheduled visits and managing care based on each patient's individual needs.
Responsibilities:
Provide skilled nursing care to patients in their homes
Complete scheduled in-home patient visits and provide care based on the plan of care
Monitor patient condition and report changes to the RN or supervising provider
Administer medications and treatments as ordered
Perform wound care and other skilled nursing procedures as needed
Reinforce patient and family education and care instructions
Communicate with the RN, physicians, and other members of the healthcare team
Document visits using Homecare Homebase (HCHB)
Requirements:
Active LPN license in the state of practice
1+ year of recent Home Health experience
BLS certification (AHA)
Homecare Homebase (HCHB) experience preferred
