Job description
Heartland Home Health is seeking a compassionate Home Health LPN to provide skilled nursing care to patients in their homes. In this role, you will perform assessments, administer medications and treatments, manage wound and catheter care, and monitor vital signs and symptoms. You will follow individualized care plans, document visits accurately, and communicate closely with RNs, physicians, patients, and families. The LPN supports patient and family education, promotes safety and independence, and collaborates with the care team to ensure high-quality, patient-centered, and mission-driven home health services.
