Job description
Elara Caring is seeking a Medical Social Worker (MSW) to join our home health team. The MSW will assess psychosocial and environmental needs, conduct initial and ongoing evaluations, and collaborate with physicians and the care team to develop and adjust the Plan of Care. Excellent communication and a valid driver's license are required.
