Job description
WellBe Senior Medical in Albion, NY is seeking a Part-time Nurse Practitioner (APP) to deliver field-based primary care in patients' homes. This role emphasizes autonomy and meaningful patient relationships.
You will conduct visits, manage chronic conditions, and coordinate with an interdisciplinary team to ensure smooth transitions and reduced ER visits. Requires NP master’s, active license, board certification, and valid driver's license.
