Job description
Optum Home & Community Care, part of Optum, is expanding its HouseCalls program to bring care to patients in their homes. The Advanced Practice Clinician role focuses on annual in-home health assessments for Medicare Advantage and other plans, with emphasis on education, gap closure, and collaboration with PCPs.
The position is field-based, allows flexible scheduling, and offers a sign-on bonus for external candidates. Excellent communication with geriatric and Medicare populations is essential.
