Job description
Optum Home & Community Care, part of the Optum family, is expanding its HouseCalls team to conduct in-home health assessments for Medicare Advantage and other members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers to improve outcomes.
As a HouseCalls Advanced Practice Clinician (NP or PA), you will perform comprehensive assessments, document diagnoses, and coordinate care with PCPs.
