Job description
Optimal Care in Michigan is seeking a Nurse Practitioner or Physician Assistant to provide primary and consultative care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to deliver high-quality, patient-centered care.
In this role you will conduct comprehensive medical evaluations, develop and manage individualized treatment plans, prescribe and monitor medications, and coordinate closely with physicians, caregivers, and interdisciplinary team
