Job description
Optum Home & Community Care is expanding a HouseCalls team to bring preventive care directly to members' homes across New York state. You will perform annual in-home health assessments for Medicare Advantage and other plans in a flexible, travel-based role with no on-call, weekends, or holidays.
The role requires NP or PA certification, active state licensure, and reliable transportation. Compensation includes base pay plus incentives, with tuition loan repayment or sign-on options available.
