nTalin Home Healthcare LLC is looking for a compassionate and dedicated Home Health Nurse to serve patients in the Culpepper area. nResponsibilities:
nProvide skilled nursing care to home health patients in the Culpepper service area
nConduct comprehensive patient assessments and develop individualized care plans
nAdminister medications, wound care, IV therapy, and other clinical treatments as ordered
nMonitor and document patient progress, vital signs, and changes in condition
nCoordinate care with physicians, therapists, and other healthcare team members
nEducate patients and family members on disease management, medications, and home safety
nMaintain accurate and timely clinical documentation in compliance with regulatory standards
nRequirements:
nActive RN license in the state of Virginia
nPrevious home health, community health, or clinical nursing experience preferred
nStrong assessment, critical thinking, and clinical decision-making skills
nExcellent communication and patient education abilities
nReliable transportation and valid driver's license for travel within the Culpepper area
nAbility to work independently and manage a patient caseload efficiently
nCompassionate, patient-centered approach to care
nMust be willing to work at least 2 days a week with 1 of those days be either Monday or Tuesday.
