Job description
Pride Health is hiring a Registered Nurse – Home Health for a contract opportunity supporting a leading healthcare organization in Indianapolis, IN .
Important Details:
- Employment Type: Approximately 14-Week Contract
- Start Date: ASAP
- Schedule/Shift: Monday–Friday | 8:00 AM–4:30 PM | 40 hours/week
- On-Call Requirement: None
- Coverage Area: Indianapolis, IN
- Mileage Reimbursement: Mileage reimbursed between patient visits
- Pay Range: $53.00–$57.00 per hour
Responsibilities Include:
- Complete skilled home health nursing visits, including OASIS Start of Care (SOC), recertifications, and discharge assessments.
- Perform comprehensive patient assessments, medication management, and wound care in the home setting.
- Maintain accurate and timely documentation using Homecare Homebase (HCHB) and other home health EMR systems.
- Coordinate care plans with the healthcare team while providing compassionate, patient-centered care.
Qualifications:
- Active Registered Nurse (RN) license.
- Previous Home Health nursing experience required.
- Experience with Homecare Homebase (HCHB) preferred.
- Strong communication skills and a positive, patient-focused attitude.
