Job description
Hartford HealthCare at Home seeks an OASIS RN to deliver skilled nursing care in patients\' homes across Connecticut. You will review referrals, perform assessments, and develop care plans in collaboration with interdisciplinary teams.
The role requires an RN license in CT and at least one year of nursing experience, with home care or acute care experience preferred. You will guide LPNs and aides while ensuring quality outcomes for patients and families.
