Job description
Matrix Medical Network seeks experienced Nurse Practitioners to perform comprehensive in-home health assessments, reviewing medical history, medications, and risk factors to identify gaps in care. Visits occur at members’ homes or SNFs and are designed to improve outcomes through targeted interventions.
Providers collaborate with PCPs and care teams, maintain detailed documentation, and travel as needed across the assigned state.
