Job description
Pathways Healthcare in Massachusetts is seeking a Nurse Practitioner to provide in-home care on a per diem basis. You will conduct admissions, assess patients, and develop personalized plans with flexibility to meet patient needs.
You’ll manage medications, perform visits, coordinate with the care team, and support discharge planning while maintaining confidentiality and high-quality care across Worcester, Sturbridge, and surrounding areas.
