Job Overview Home-based and community-focused care model
Monday-Friday schedule
No weekends or overnight call
Blend of in-home visits and virtual care
Collaborative interdisciplinary team with strong clinical support
Opportunity to care for patients with chronic and medically complex conditions
Established patient population with continued program growth
