Job description
Pride Health is hiring a RN - Home Health/Case Manager to support our client's healthcare facility in Uniontown, OH .
Job Details:
Schedule: 8:00 AM-4:30 PM | 36 Guaranteed Hours/Week | On-Call/Weekend: 1 Week and 1 Weekend
Location: Uniontown, OH
Coverage areas include Summit, Portage, and Medina Counties
Mileage reimbursed: Yes
Job Type: Contract
Contract Length: 13 Week/s
Pay Range: $52-$54
*Pay offered is based on experience, expertise, credentialing, and education.
Duties:
- Manage and coordinate patient care as a Case Manager.
- Assess patient needs and develop individualized plans of care.
- Coordinate services and communicate with physicians and interdisciplinary healthcare teams.
- Complete accurate and timely patient documentation and follow-up.
Requirements:
- License(s): Active RN license in Ohio
- Certification(s): BLS
- Years of experience: 1+
- Skills/Specialties/Technology: HCHB experience preferred
