Job description
Lifeline Home Health, part of LHC Group and the Optum family, seeks an Occupational Therapist to evaluate patient needs in home health and develop therapy plans. You will assess functional status, daily living activities, and equipment needs, coordinating with the health care team.
You will work in diverse home settings, document progress for CMS, and support patient independence. Excellent communication and organizational skills are essential in a patient-centric environment.
