Job description
Commonwealth Home Health, part of LHC Group within the Optum family, seeks an Occupational Therapist in Home Health to assess and address functional status, ADLs, and equipment needs in patients’ homes. Full-time and PRN positions are available with a compensation structure including per-visit pay and hourly components.
Responsibilities include evaluating function, planning care with physicians, and documenting CMS information. A valid OT license and reliable transportation are required.
