Job description
TriCounty Home Health, part of LHC Group and within the Optum family, is seeking an Occupational Therapist to assess and evaluate patient care needs related to ADLs, functional status, and home safety. You will plan care, coordinate with the health care team, and support patients and families in their homes.
You'll be responsible for CMS documentation, adapt equipment, and ensure care plans align with state laws.
