Job description
Geisinger Home Health, part of the Optum family, is seeking an Occupational Therapist to assess and address functional status, ADLs, and home safety needs in home health settings.
You will work with a multidisciplinary team to develop and implement an evidence-based plan of care, evaluate progress, and adapt interventions. Strong communication and organizational skills are essential for coordinating care with patients, families, and providers.
