Job description
Assisted Home Health & Hospice in Anthem, AZ is seeking an Occupational Therapist to join our home health team. You will conduct initial and ongoing assessments to determine function and response to therapy, and you will develop plans of care to relieve pain and restore independence.
Collaborating with physicians and an interdisciplinary team, you will educate patients and families, implement self-care modifications, and assess equipment needs ordered by the physician.
