Job description
CenterWell Home Health is seeking an Occupational Therapist to deliver compassionate, in-home therapy to seniors and adults with chronic conditions. In this field-based role, you will conduct comprehensive assessments, create individualized treatment plans, and provide hands-on interventions that improve safety, function, and independence at home.
