nPosition Details
nOurVisiting Occupational Therapistfor our VNA Certified division is responsible and accountable for evaluation, coordination, and administration of quality occupational therapy services.
nAt Aveanna, our services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients’ homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs.

nWhat You'll Do
nnnFollow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws
n
nnProvide occupational therapy services as prescribed by the physician
n
nnProvide a written evaluation defining occupational therapy needs, goals and expected outcomes in conjunction with referring physician's Plan of Treatment
n
nnProvide a written Plan of Care from the nursing staff to provide continuing care
n
nnCarry out appropriate techniques according to identified goals and treatment programs
n
nnParticipate in coordination of services (including discharge planning), with health care team, physicians, parent/guardians, nurses, and appropriate others
n
nnAdminister care to ensure safety for the patient
n
nnUpdate self on the patient's clinical condition
n
nnMaintain effective communication between staff, health care team members, and family.
n
nnServes as a resource consultant for clinical staff
n
nnProvide physicians and Aveanna with written evaluation of patient's needs, goals, and progress on a weekly basis or as ordered
n
nnAttends patient case conferences when requested. Attends and participates in in-services as indicated.
n
nnSupervises other personnel (COTA, HHA) when required
n
n nQualifications
nnnA certification in Occupational Therapy in the state of application
n
nnCurrent and unrestricted license in the state of practice
n
nnCurrent CPR certification (with hands-on component) or able to obtain one
n
nnCurrent TB test or able to obtain one
n
nnMust have a drivers license and reliable transportation
n
