Job description
Humana CenterWell Home Health is seeking a licensed Occupational Therapist to deliver home-based rehabilitation, assess function, and develop retraining plans for patients in their homes and communities. You will coordinate with physicians and the care team to implement plans that improve strength, mobility and independence.
Responsibilities include documenting progress, participating in discharge planning, and training staff and family to support ongoing rehabilitation.
